Key Responsibilities

• Conduct comprehensive assessments of patients’ health status, needs, and social determinants of health.

• Coordinate with interdisciplinary teams, including physicians, therapists, and social workers.
• Monitor patients’ progress and adjust care plans as needed.
• Educate patients and their families about health management, resources, and care options.
• Maintain accurate and up-to-date patient records and documentation.
• Provide telephonic support to patients and caregivers.
• Advocate for patients’ needs within the healthcare system.
• Participate in case conferences and team meetings.

Qualifications

We are looking for talented and dedicated professionals who meet the following criteria:

• A current and valid Registered Nurse (RN) license.
• At least 2 years of clinical nursing experience.
• Experience in case management and/or home health preferred.
• Excellent communication and interpersonal skills.
• Strong organizational and critical thinking abilities.
• Ability to work independently and manage your own schedule.
